Sound Health Physicians has been working with Quest Diagnostics, the largest provider of laboratory services in the nation.  With this partnership, Sound Health Members can access their medical information through the Quest Diagnostics portal.  So you can connect with your medical information simply request a password from us; you can call or email.  Then, follow these simple steps:

 

1. Create a PatientPortal account
Visit http://myquest.questdiagnostics.com/web/ and create an account if you don’t already have one. To confirm that you have created this account, we will send you an email that you will need to access and validate your request to create an account before you can login the first time.
2. Connect to your physician
Once you login, navigate to the Lab Results option and select to connect with your physician. Then, enter the entire PIN (that you request from Sound Health Physicians), your last name, and date of birth, to connect to your physician. (Your last name and date of birth must match exactly what is on file with your physician).
3. Access your Personal Health Information
Once step 2 is successfully completed, you can now begin to receive your health information from your connected physician. You can also send secure messages to your physician regarding your health, or to request additional health information. Lastly, you can share your health information with other physicians that are part of your care team.
